To provide optimal comfort and dignity to our patients through their end-of-life journey. We will achieve this by concentrating on patient and family centered goals with a focus not only on the medical needs but also by providing care for the mental and spiritual needs of our patients and families during their journey.
Who We Are
The word Solace came from the Latin verb “solari” meaning “to console”. In English it means something that gives comfort, consolation or relief. Solace Hospice is a hospice care center situated at Concord, California, certified by Medicare. Solace Hospice vision started on 2019 and got certified by CMS in 2020. This is the vision of 5 nurses whose passion is to provide quality care by providing an all-encompassing supportive service to patients, their families, and loved ones in both home and inpatient settings. This includes addressing the spiritual, emotional, physical, and social care needs of an individual in the last stages of illness. We are committed to achieving a team-oriented approach thus, our vision and passion were poured into the company of Solace Hospice.
